Heaven Sent provides services for persons with developmental disabilities. Our sole purpose is to help our residents become more independent in the community by offering:

• Public Transportation with a 20 Mile Radius of Salt Lake City, UT

Our services are designed to support the following types of disabilities

​

• Brain Injury-an acquired injury to the brain that is neurological in nature. This would not include those with deteriorating diseases such as Multiple Sclerosis, Muscular Dystrophy, Huntington's Chorea, ataxia, or cancer, but would include cerebral vascular accident

• Person's with mental & physical disabilities Other conditions-a severe, chronic disability that meets the following conditions: (1) Cerebral palsy or epilepsy, or any other condition other than mental illness found to be closely related to mental retardation (becuase this condition results in impairment of general intellectual functioning or adaptive behavior) (2) it is manifiest before the person reaches age 22 and it is likely to continue indefinitely, and (3) it results in substantial functional limitations in three or more of the following areas of major life activity: self care, language, learning, mobility, self-direction, and capacity for independent living

• Legal Status-Heaven Sent only serves persons with disabilities who are funded by the state of Utah. Thus, the legal determination or status for these clients does not dictate placement of the clients as this determination, and the funding associated, is determined by the Department of Health and Human Services

• Age & Sex-Heaven Sent, LLC serves persons who are eligible for services under the contract it holds (DHS 90743) Then the age and sex do not determine the placement. Thus Heaven Sent, LLC will serve clients of any age and sex. However, the primary client tends to be young adults or adults with intellectual disabilities. 

• Program Limitations-Our program is limited to serving people who qualify for services through DHHS or private pay clients who share a significant commonalities with our target popuation

• Day Treatment-Day treatment will serve individuals with disabilities as listed above. Underage clients (age 18 or younger) must be approved by the person's support team, and follow all rules as applicable.
